The Position
Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents possess a degree of knowledge and proficiency sufficient to perform work independently with little or no additional training. The Aging and Disability Services Division (ADSD) is currently recruiting for a full-time Health Care Coordinator 2 in Reno/Sparks, Nevada. This recruitment may be used to fill current and future vacancies as they occur in the following Program: Home & Community Based Services (HCBS).
Incumbents oversee caseloads of program participants based on slot allocation, staffing ratios and manage a caseload of participants residing in urban/rural areas. To offer participants with case management, incumbents will evaluate each person and set up continuing services in accordance with Person Centered Plans of Cares, monitor program services, and help or refer clients to the proper community resources. Incumbents will contact clients via phone, email, and/or in-person interaction. Find and organize a network of resources and services to address the health and safety requirements of the participant. Promptly enter program data into the appropriate databases and submit the necessary reports to the appropriate state, county, or local government agencies. To Qualify:
In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
    Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment, and one year of professional experience providing case management services in a social or health related field; OR one year of experience as a Health Care Coordinator I in Nevada State service; OR licensure and an equivalent combination of education and experience.
Special Requirements
    A State of Nevada/FBI background check will be required of the selected applicant.
    Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada is required at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
    This position has a 50% travel requirement.
    This position requires a valid driver's license or evidence of equivalent mobility at the time of appointment and for continuing employment.
