The salary range indicated in this announcement is based on the Employee/Employer retirement compensation schedule. An employer-only option is available at a reduced salary range. Generally, salaries start at the lowest level of a step 1 and increase with each year of service to the highest level of a step 10.

JOB INFORMATION

Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation. Aging and Disability Services Division (ADSD) is currently recruiting for multiple Health Care Coordinator I positions in Las Vegas, NV. This recruitment will be used to underfill positions within the Health Care Coordinator series. Advancement to the next level may occur after meeting the minimum qualifications, satisfactory performance, and with endorsement of the appointing authority. This recruitment may be used to fill current and future vacancies as they occur in the following Programs: Home & Community Based Services (HCBS) and Facility Outreach Community Integration Services (FOCIS)/Money Follows the Person (MFP).
ADSD positions assigned to the HCBS ensure a coordinated support system to meet the needs of at-risk elderly and people with disabilities by providing case management and essential services to recipients of HCBS programs statewide to facilitate community integration and avoid institutionalization. Responsibilities include case management, assessment, eligibility determination, development, implementation, oversight of the plan of care, home visits and documentation utilizing a web-based case management software system.

In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment.
Special Requirements
A valid driver's license is required at the time of appointment and as a condition of continuing employment.
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
