Social Workers are licensed professionals who provide case management services to children, families, vulnerable adults with intellectual, developmental, physical disabilities, serious mental illness, and older adults (collectively referred to as "vulnerable adults") and other individuals within local communities and institutions.

Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation. The incumbent will underfill the Social Worker 2 position and will advance to the next level in the series upon recommendation of the Appointing Authority, meeting the minimum qualifications and satisfactory performance. This position will be primarily responsible for providing case management for recipients in the Home and Community Based Waiver (HCBW) program for the Frail Elderly (FE) and Persons with Physical Disabilities (PD). This includes interviewing and assessing individuals to determine if the criteria are met for placement on the HCBW and providing case management through other Medicaid waivers, and State Funded programs such as Personal Assistance Services (PAS), Community Options Program for the Elderly (COPE) programs and Homemaker Program. This position will complete monthly contacts with program recipients, complete required reports to appropriate agencies, complete documentation in set databases and complete community program referrals as needed.
