Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents possess a degree of knowledge and proficiency sufficient to perform work independently with little or no additional training.
This position is located within the Aging and Disability Services Division (ADSD) of the Department of Health and Human Services, Community Based Care program (CBC) in Las Vegas, NV. ADSD provides an atmosphere of innovative thinking, teamwork, friendly co-workers, and promotes a positive work environment. The mission of the Aging and Disability Services Division (ADSD) is to provide leadership and advocacy in the planning, development, and delivery of a high quality, comprehensive support service system across the lifespan to allow all of Nevada's elders, children and adults with disabilities, or individuals with special health care needs, to live independent, meaningful, and dignified lives to the greatest extent possible.
Positions assigned to the Community Based Care Program ensure a coordinated support system to meet the needs of at-risk elderly and disabled persons by providing case management and essential services to recipients of CBC programs statewide to facilitate community integration and avoid institutionalization. Responsibilities include case management, assessment, eligibility determination, development, implementation and oversight of the plan of care, home visits and documentation utilizing a web-based case management software system.
In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment, and one year of professional experience providing case management services in a social or health related field; OR one year of experience as a Health Care Coordinator I in Nevada State service; OR licensure and an equivalent combination of education and experience.
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
A State of Nevada/FBI background check will be required of the selected applicant.
Requires a valid driver's license or evidence of equivalent mobility at the time of appointment and for continuing employment.
