Social Workers are licensed professionals who provide case management services to children, families, vulnerable adults with intellectual, developmental, physical disabilities, serious mental illness, and older adults (collectively referred to as "vulnerable adults") and other individuals within local communities and institutions.
Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation.
This position is underfilling a Social Worker 2 and is located within the Aging and Disability Services Division (ADSD) of the Department of Health and Human Services, Community Based Care unit in Reno, NV. ADSD provides an atmosphere of innovative thinking, teamwork, friendly co-workers, and is a great place to work. The mission of ADSD is to provide leadership and advocacy in the planning, development and delivery of a high quality, comprehensive support service system across the lifespan to allow all of Nevada's elders, children and adults with disabilities or special health care needs, to live independent, meaningful and dignified lives to the greatest extent possible. This position will be primarily responsible for providing case management for recipients in the Home and Community Based Waiver (HCBW) program for the Frail Elderly (FE) and Persons with Physical Disabilities (PD). This includes interviewing and assessing individuals to determine if the criteria are met for placement on the HCBW and providing case management through other Medicaid waivers, and State Funded programs such as Personal Assistance Services (PAS), Community Options for the Elderly Program (COPE) programs and Homemaker Program. This position will complete monthly contacts with program recipients, complete required reports to appropriate agencies, complete documentation in set databases and complete community program referrals as needed.

Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker or Associate in Social Work. |

A valid driver's license is required at the time of appointment and as a condition of continuing employment. |

A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for fingerprinting. |

Licensure or provisional licensure by the State of Nevada Board of Examiners for Social Workers as a Social Worker, Clinical Social Worker, Independent Social Worker, or Associate in Social Work is required at the time of appointment and as a condition of employment. |

The exam will consist of an application evaluation. It is essential that applications include detailed information with time frames regarding education and experience. The most qualified applicants will be contacted by the hiring agency for interview. The hiring agency may require specific skills related testing as part of the interviewing process. |
