The Position
Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents possess a degree of knowledge and proficiency sufficient to perform work independently with little or no additional training.
This position is located within the Community Based Care (CBC) Program of the Aging and Disabilities Services Division (ADSD) of the Department of Health and Human Services in Las Vegas, NV. ADSD provides an atmosphere of innovative thinking, teamwork, friendly co-workers, and provides a positive working environment. The mission of ADSD is to provide leadership and advocacy in the planning, development and delivery of a high quality, comprehensive support service system across the lifespan to allow all of Nevada's elders, children and adults with disabilities or special health care needs, to live independent, meaningful and dignified lives to the greatest extent possible. This position provides specialized instruction and case management services to children under the age of three who have or at risk for developmental delays, disabilities (including autism, vision and hearing impairments, genetics syndromes and prematurity) under the federal regulations of Individuals with Disabilities Education Act (IDEA). Also, serves on multi-disciplinary teams to determine program eligibility and services. Aging and Family Services Division positions assigned to the Community Based Care Program (CBC) ensure a coordinated support system to meet the needs of at-risk elderly and disabled persons by providing case management and essential services to recipients of CBC programs statewide to facilitate community integration and avoid institutionalization. Responsibilities include case management, assessment, eligibility determination, development, implementation and oversight of the plan of care, home visits and documentation utilizing a web-based case management software system.

To Qualify:
In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
    Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment, and one year of professional experience providing case management services in a social or health related field; OR one year of experience as a Health Care Coordinator I in Nevada State service; OR licensure and an equivalent combination of education and experience.
Special Requirements
    A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
    Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
    Requires a valid driver's license or evidence of equivalent mobility at the time of appointment and for continuing employment.
