Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
This position is located within the Aging and Disability Services Division (ADSD) of the Department of Health and Human Services - Community Based Care (CBC) in Las Vegas, NV. Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation. This is an underfill for the Health Care Coordinator 2 level. Aging and Disability Services positions assigned to the Community Based Care Program (CBC) ensure a coordinated support system to meet the needs of at risk elderly and disabled persons by providing case management and essential services to recipients of CBC programs statewide to facilitate community integration and avoid institutionalization. Responsibilities include case management, assessment, eligibility determination, development, implementation and oversight of the plan of care, home visits and documentation utilizing a web based case management software system.

In order to be qualified, you must meet the following requirements:

Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment. |

A valid driver's license is required at the time of appointment and as a condition of continuing employment. |
|
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items. |

Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment. |

Travel up to 50% of the time is required. |
