JOB INFORMATION
Work Type: Permanent, full time vacancies as they may occur
in this geographical location.
Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation.
The Health Care Coordinator I serves within the Division of Health Care Financing and Policy (DHCFP/Medicaid) of the Department of Health and Human Services. Duties will fall primarily under the Facility Outreach and Community Integration Services (FOCIS)/Money Follows the Person (MFP) Unit located at the Las Vegas Medicaid District Office. FOCIS is a program created by the State of Nevada in response to the Federal Olmstead Decision. This mandate requires that individuals residing in nursing facilities, hospitals, or rehabilitation centers, be given the opportunity to explore resources as an alternative to living within an institution.
MFP is a grant awarded to the State which provides qualified individuals residing in long term care settings resources to assist them in moving out of institutions into their own homes or community based settings. MFP's goals are to rebalance Nevada's resources to provide the opportunity for every Medicaid nursing facility resident to live in their own home in the community.
Health Care Coordinators, within this unit, perform professional work related to Medicaid program operations which includes research, assessment, and care plan development in compliance with program policies and regulations. Health Care Coordinators are out in the field, meeting with recipients, or within travel up to 50%. In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment.
Special Requirements
A valid driver's license is required at the time of appointment and as a condition of continuing employment.
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
A State of Nevada/FBI background check will be required of the selected applicant.
Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
Travel up to 50% of the time is required.
