JOB INFORMATION

in this geographical location.
Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.
Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation.
This position is located within the Department of Health and Human Services/Division of Health Care Financing and Policy/Elko District Office. This is a Health Care Coordinator II position underfilled as a Health Care Coordinator I position. The incumbent will primarily responsible for providing case management for disabled recipients under the age of 19 years old who are eligible for the Katie Beckett Eligibility Option as determined by the Division of Welfare and Supportive Services. Case management includes assessment of level of care (LOC) for new referrals and redeterminations, oversight and coordination of services, contact with providers, monitoring quarterly costs and a minimum of one home visit annually. The position requires quarterly telephone contacts with the recipient's parent or legal guardian. The incumbent will also be responsible for case management for recipients in the Facility Outreach and Community integration Services/Money Follows the Person (FOCIS/MFP), Care Coordination, Serious Occurrence Report (SOR) evaluation and follow up. The incumbent must be able to assess an individual's needs for Medicaid services and link them to appropriate community resources in order for them to live and / or be cared for in a community setting. The travel requirement for this position is approximately 20% for the Elko District Office service area.
In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment.
Special Requirements
A valid driver's license is required at the time of appointment and as a condition of continuing employment.
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
A State of Nevada/FBI background check will be required of the selected applicant.
Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
