The Position
Health Program Specialists plan, develop, implement and evaluate health services and activities; develop goals, objectives, program requirements and procedures; provide technical information regarding specialized health related conditions; and participate and provide support to various committees, advisory boards and other work groups.
Incumbents function as first-line supervisors who train, supervise and evaluate the performance of assigned staff; assign and review work; and initiate disciplinary action.
This is a Health Program Specialist 2 position within the Chronic Disease Prevention & Health Promotion (CDPHP) Section in the Bureau of Child, Family, and Community Wellness. The overall purpose of the position is to manage all Health System Inverntion functions including: grants, reports, evaluation of program activities, community collaborations, and programmatic budget activites. This position is responsbile for the Community Health Worker (CHW), diabetes and heart & stroke programs. This position oversees all activities pertaining to the 1305 grant including data analysis, reporting and evaluation related to wellness efforts by a) promoting reporting of hypertension and diabetes screenings; and, as able, initiate activities that promote clinical innovations and team-based care; b) promoting awareness of evidence based wellness initiatives among high-risk populations; and c) increasing implementation of quality improvement processes in health systems by increasing electronic health records (EHR) adoption and the use of health information technology (HIT) to improve performance and standardized quality measures at the provider and systems level. This position leads communication efforts with the CDC-assigned project officers, assists in writing continuation grants and progress reports associated with funding and speaks and presents publicly to stakeholders.

To Qualify:
In order to be qualified, you must meet the following requirements:
Education and Experience (Minimum Qualifications)
    Bachelor's degree from an accredited college or university in a health science, behavioral science or closely related field applicable to the program area and three years of health-related experience which included researching, collecting, and analyzing data and statistics, developing objectives, protocols, and procedures, providing health education services to target groups or the general public, and/or investigation and prevention of communicable or chronic diseases; OR an equivalent combination of education and experience.
Special Requirements
    A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for these items.
